DOECL investigation services are carried out in order to determine cause(s) of an accident, incident or near miss.
Our programme examines each stage in the management system by measuring compliance with the controls the organisation has developed.
The outcome is to prevent further happenings of a similar kind. DOECL can also provide training to help you develop a standardized technique.
The investigation services are often performed in four steps:
- Fact gathering
- Fact Analysis
- Conclusion Drawing
Examples where investigations are useful:
- Accident (undesired event that results in personal injury or property damage)
- Incident (unplanned, undesired event that adversely affects completion of a task)
- Near miss (where damage and/or injury easily could have occurred)
